    Have you set an appropriate level for PHP error reporting?  You 
might set

error_reporting = E_ALL

while you are trying to sort this out, if you don't already have full 
reporting enabled.

Also, is it possible that your   "upload_max_filesize"  value is too 
small for what you
are trying to do?
