[support] Uploaded files "falling out" of the system

No it does not happen to ALL files, it seems random.

File types are allowed (last file that fell out was a rtf document

Drupal 4.6.3

Standard upload module

Temp directory is CHMOD 777

Anna,

  I think we need more info on this...

  Does this happens to all uploaded files?
  Does the file type (extension) is included in the allowed upload files?
  What drupal version are you using?
  Are you using the default upload module?
  What is the setting on the temporary directory?

Watchdog reports no errors, the files simply vanish from the database.
