I swear by Pajunas Interactive:  www.pajunas.com

But they're not for everybody in the world.  If most of one's intended 
audience were located in Asia, for example, one would want to host 
geographically close to that audience to reduce network latency while they 
browse.  Thus, hosting at a company in North America might not be the best choice.
