 Did you re-apply the menu per role patch per the readme instructions? 

 

It seems like you might not have.  This must be done after every
upgrade.

I tried completely deleting the three menu items and re-creating them,
restricting them as I had before with Menu Per Role module, but they are
all still available to the anonymous user.

Is there some way I can completely reset all my permissions?  Even if I
have to delete everything and start over, I'd rather do that than have
too much access (as I do now).
