[support] Fwd: Image captcha refresh issue

Is it a known issue in Captcha ?

Dear All
I am using Image captcha refresh module in D7.15 for generating new
captcha. For most of my forms it worked fine. For two forms it is not
working. Listing down problems below.

1. After installation of captcha, installed image refresh captcha module.
Enabled "Add CAPTCHA administration links to forms" in captcha
configuration. For most of the forms, it showed me link "Show CAPTCHA: no
challenge enabled" when I logged in as admin user. I enabled captcha using
that link for all forms.

2. For two forms it showed "Show CAPTCHA: no challenge enabled", but it was
a plain text, not a link. I could not enable captcha usual way. I debugged
using firebug and found the form id. Manually in captcha configuration I
set the captcha using that form id. It was successful.

3. Next I logged in as a normal user. I am able to see captcha in those two
forms for which I set captcha manually using form id. But if I click
generate new captcha I get some text in page as below, captcha is not
refreshed.

I am not sure what's going wrong. My environment details are

Drupal 7.15
Image refresh captcha - 7.1.5
Captcha - 7.x-1.0-beta2 <http://drupal.org/node/1391674>

Can somebody point me whats going wrong here, how can I resolve this.
