There is some erratic bevhavior with with fileapi_unique_name when repeated switching private state. I think there are two bugs at play here... 1) data stored in form not being properly updated up actions (unset/set element values in action switch). 2) $dst array building using basename($src['path']) when src[name] or dst[name] is available.
