It was my base url after all.  I didn't put the www subdomain in front of the URL, but I was accessing the website from www.example.com, not just example.com.

I think this is because you have PHP in safe mode. It will then not return any
errors. While you may have such errors.

Look in your error logs please, and paste any Drupal/PHP related errors.
