One of the sites I manage didn't take Drupal 5>6 well. This is my 3rd site doing this, always follow the posted instruction carefully, and nothing was out of ordinary. I got so many duplicated (or some of them are triplicated) admin menu items outside of Administer menu. If I install a new module, it shows up under Navigation menu instead of under Administer menu. What this means is that it won't show up under http://mysite.com/admin (I didn't know they were directly linked). Ran update.php like a mad man. Emptied every possible caches manually but to no avail. Optimize DB using DB Maintenance module but no effect. Then I noticed more than half of the menu items have "Reset". So I clicked them all. Every time I click Reset, the shown menu items changes inconsistently. I went along and clicked them all, only to find more than half of what supposed to be are now gone. Running update.php put me right back to where I started: duplicated and triplicated admin menu items. I posted this here a few weeks ago but didn't get any response.
