[development] update/replace the book module - poll, questions

Tao, this is exactly the point we're heading for- the new "book"
module should primarily provide a UI and navigation elements, while
all the heaving lifting in terms of maintaining the hierarchy will be
done with the code and tables defined in menu.inc.
