[development] User Password Hashing

There's been a rather long discussion of password security related to the
hash algorithm.  Given that this discussion began during the 5.x push and
given its security implications, I'm risking breaking protocol and asking
for a review.  The thread has reached some agreement, but there are some
implementation questions, one of which can really only be answered by a
handful of people.
