Hi,
I want to do the same. but i dont know where to use the javascript code for
form-api of select-list. Can you please give me small code example for where
to write the javascript in FormAPI?.

The requirement is to bring the data record from database table and fill the
form with respected fields when i select dropdown(select-list).

Im not sure what are you talking abotu exactly but try to put javascript
into the select - onchange="submit();"
