To (i) fix this and (ii) to be able to compile a list of people who 
contributed to a book page, we need better content versioning.  In 
particular, we need to put some time, effort and love into this patch:

It is much needed, so I'll spend some time reviewing the patch and 
formulating recommendations as how to move forward.  I won't e-mail 
these to the documentation mailing list so check the issue [1] if you 
want to follow up on that.
