This is a great list of questions. It looks simple enough that the
survey.module can handle it...so I would suggest we use that in the
spirit of eating our own dog food.

I'll happily put this on our support site if desired. Or, Kieran, you
should have it enabled at CivicSpaceLabs as well.

(Ideally this would be part of a *.drupal.org site; maybe
drupaldocs.org for now, with a little hack to send people back to the
referring page when they are done/submitted?)

Very cool! Just checked it out briefly, will look at it and go through
