very old generic issue, closing as part of queue clean up as much has
been done in the last two years.
Please open a new issue with links to specific pages rather then
re-open this.

Currently php code chunks should use the php open and close tags to get
proper formating.  We need to go through and clean up code chunks in
the handbook.  I will take a stab at this across all handbook pages.

Note that I recently added support for <?php ?> inside <code> tags.
Using code tags when they are unnecessary is obviously silly, but when
mixing HTML and PHP it is essential.
