[documentation] New Forms API tutorial

Typically "credit" for who wrote or added to a handbook page is  
handled by the revisions tab where folks can see the whole history of  
users. That can be seen by all users and keeping it there makes the  
article itself much cleaner while still letting people see who did  
what. I'd prefer that we stick to that convention.
