It seems that you want the functionality that exsits in Taxonomy Other
http://drupal.org/project/taxonomy_other

But I do not know if you can use taxonomy with profiles.

I'm not experienced enough w/ drupal yet, to know if a drupal module
exists, but you CAN do all of this with css/jscript. You might consider
doing it in jquery lingo. There are many good web resources for this,
like http://css-tricks.com -- which I know has a tutorial and screen
cast on building conditional cascading forms etc.
