As far as your own modules are concerned, you can get an overview of the API
changes and what that would entail at: http://drupal.org/node/224333 -
obviously the complexity of your custom module code will be the determining
factor here.

For contrib modules, this will depend heavily on how many you are using and
which.  Many of the major ones have made a commitment to have a release
ready when Drupal 7 is released.
