This is pretty simple, but it requires you have an element with an id
towards the top of the page. For example, your page title may have the
id "title" or your center region may be have the id "center". If you
have no such element, you can create one with a block:
Administer > Site Building > Blocks > Add Block
Description: Id
Block Body: <span id="top-of-page"></span>
Input format: Full HTML
You can then place this Id block at the top of your header region.
To make the [top] link is a very similar process
Administer > Site Building > Blocks > Add Block
Description: Top Link
Block Body: <a href="#top-of-page">[Top]</a>
Input format: Full HTML
Note that "top-of-page" should be whatever id you found towards the top
of the page.
Then place this block in the Footer region.
You'll probably want to play around with block visibility (to limit it
to pages with content on them rather than all pages), but this should
give you a start.
