I found how to recreate this.
Using the file_sources and file_paths module.

1. create content
2 I click on file attach as I am copying files from my server
Now just click save and you will get this error.
If you click the attach button then save all works fine.

The last site I built and I still can do this is I can just click the save
button and all works fine.
