I'm not saying your file-rights are not set properly. I was saying that *IF*
not set properly the theme-listing will be affected.
In your case the file-rights seem OK.
Since I have a lot multisite setups and run into issues sometimes, I tried
to change things in order to reproduce your error, with no luck.
You said the mysub.mysite.com works properly, so the settings.php file is
found properly and the specifity of the mysub.mysite.com-directory is OK.
Would you mind reporting back what the solution was?

I don't think so. The owner:group are both apache like everything else, and
the permissions on the themes folder and the theme are 755 just like in
sites/all/themes. If I mv mydomain/themes/mytheme all/themes it appears in
the list (even without a cache clear). If I do it in the other direction, it
disappears, even with a cache clear. I have tried making the permissions
777, and the behavior doesn't change.

Should be the correct location indeed. I have checked this in my own
installation.
The only thing I can think of is the file-rights. I have testes this too.
With file rights not set properly the theme will not listed.

I have a site mysub.mysite.com, a D7 site, set up as multisite.
I have its sites directory at sites/mysite.com.mysub and its settings are
found and work.
In that directory I have a themes directory, and in it, a mytheme directory.
Having cleared cache via the admin panel and drush, mytheme is not shown in
/admin/appearance. If I move the theme to sites/all/themes, it is listed.
