Don't think this belongs here, but ...

Are you running PHP as FastCGI? I have seen this happens when using
mod_fastcgi or mod_fcgid. Happens only the first time php-cgi is spawned.
Hitting F5 makes it work fine (probably because APC has parsed and cached
all the 100+ modules that you have on the site).
