[support] Missing Forum categories box

In addition to the original problem description, I've also noticed
that for entries made without the box, the breadcrumb only says
forums, no categories showing after that at all.  I tried to find the
time frame for when this happened. It appears to have occurred around
the time when, in trying to troubleshoot another problem, I had
disabled then re-enabled all modules, including the forum module.  I
wonder if that could have something to do with this happening? I found
a forum message on Drupal.org mentioning something about this, but
they had no lists at all for the forums area. Not sure if that's
relevant or not.

These are cleaned up entries from the modules list and have version
info as well as their purpose and dependencies are there also. Sorry
it's so long.
