[development] a more flexible book module

I wrote the list before about proposed changes to the book module (http://drupal.org/node/65319). In response to comments a number of developers and book module users, I've posted a new issue and am trying to take this the last steps before the freeze: http://drupal.org/node/81226 I would deeply appreciate your (public or private) comments on both the code and the concept.
