I don't see anything wrong with /default/. Most installs, I suspect, are single-site. Let's not make things too complicated for single-install users unnecessarily. /sites/all doesn't require removing /sites/default, which is still very useful.
Right on the head. The differentce between them should be default - a site actually, all - a prototype or in OO speak the parent class for all sites.
