Historically, we decided not to use alter tables to add new fields. This results in many, many JOINs, combined with some node caching algorithm. People who can't alter tables, can't run the upgrade script so it is probably safe to assume that 99% of all Drupal sites can alter their tables. Therefore, I'd like to open up this discussion again.
