I just want to second the custom 403 handler approach. I have done that before, and the menu query check would be easy (check the $_GET['q'] against {menu_links}.link_path and account for aliasing. menu_tree_all_data(). I used this same function when I ported http://drupal.org/project/dhtml_menu to Drupal 6.
