Just as blocks positioning is not hard coded into the theme, content positioning shouldn't be hard coded into the theme. The stuff that belongs in the theme, xhtml and css, stays in the theme as the code for page layouts.
Actually - block positioning is very often 'hard coded' (I prefer the term 'modified') in the theme. Module and UI say position this block [say] right.
