In this commit we are removing the automatic creation of a default donation form. Also including a change to the admin menu handling, adding a List menu item to te admin view as a MENU_DEFAULT_LOCAL_TASK. Finally, we're wrapping an additional callback to ensure that the pay_form table exists prior to generating menu items, this removes the potential for an error state during module uninstall.
