[consulting] Can't add new choices on Poll

Hello. I was just wondering if anyone knows the solution for the attached
error in Drupal.
It actually happened when I tried adding new choices when creating a poll.
I tried to look on Drupal.org but as usual, I  did not find any helpful
solution.
