[documentation] Contributed modules tagged as 4.6 and comments added to documentation

Hi, I went through all the contributed modules and tagged them at  
4.6.   I also cleaned up a number of comments and incorporated their  
comments into the documentation.

There are also two authors who have been adding documentation for  
contributed modules but not following the guidelines.   This is  
reducing the quality of the list.   I have contacted both authors and  
made some significant edits to their posts.  But I am inclined to  
unpublish documentation that consists of a single sentence or that  
lowers the quality of the module documentation significantly.

Hopefully the authors will review the edits I have made and add  
context sensitive links to their module in a "You can" section.
