[documentation] Help w/ drawing a tree

Should I name names? Okay, Dub explained this to me when he approved a 
couple snippets I posted, and my pages showed up on the list with him as 
"author." It seems the column is mis-named and should be called 
"approved by" or the column is misbehaving.

I'm aware that whether to provide authoring info to handbook pages has 
been batted back and forth here. I just wanted to point out that the 
little information we have is not quite what it seems.

FWIW, the page count in the author list page does seem to increment 
appropriately.
