Since I was hoping to start working tonight, I went ahead and replaced with img_assist-5.x-1.1 on my local server to see if it works.
The good news is the image browser is back, and all the missing images showed up. The bad news is I still can't place inline image since it completely drops img tag and its path.
I am going to bed for now. Hope someone has a good suggestion soon :-)
The dropped img tag sounds like the wrong ``input filter'' is applied.
