I sent you a screenshot of my admin/banner page. Is that what yours looks like?
Note that on the right hand side is an "options" column with a "view" and an "edit" link which should contain a page where you can "enable" a specific banner.
I had a problem yesterday where a block on the right side of a themed site was placed on the top of content on the page and was located in a specific pixel location within the page. This meant that if certain items in the body were really wide they would get hidden under this block. My solution was to go to blocks/configure and limit the block to be hidden on admin/* pages. I'm not sure if that is the problem in your case, but it's a useful hint in general anway.

When I look under admin->banners, it shows the banners with no options other than to list them and sorting options. I can click on the user that submitted them, the admin, but that just takes me to the user permissions. There isn't a place under admin->banners to add a banner.
Is there something not correct in the banner table possibly?

You approve a banner by editing it in your admin screen and change its status.

There seems to be an issue with approving the ad after it get submitted.
An e-mail is sent to the administrator account that has a link that wont resolve, http://mydomain.com/banners/view/all/pending which is not valid. And, I cannot see in the administration how to "approve" it.
I did not see as part of the instructions in the INSTALL file to set any url locations.
Should I check out what is in cvs ?
