I had the same problem when I start with this module.

You need to configure for which role the menu entry is visible and which not (at the bottom of the access rule).

When I try to use menu_per_role, some abnormal behavior start appear. I set one item for a role and it appear for other role not for that I has assigned. I try to recalculate the permissions of my content, clean the menu cache, rebuild the menus, etc... Not of these resolved my problems.
