I do have all following "book module" permission enabled: (for the user that belongs to the role that I login under)
create book pages create new books edit book pages edit own book pages outline posts in books see printer-friendly version
Also, just to clarify in my mind, I've also logged in as Admin and I don't see the tab. Am I wrong in thinking that the Admin has "Permission" to access "ALL" functionality?
As I side note, I "think" I know what the tab presentation should look like because on my friends Drupal install we do see the Outline tab, but we can not figure out what's different on my install? Also, my friend does not recall doing anything special to get his tab to show up other then just turning on the Book module.

I think you need the "outline posts in books" permission.
But, when I edit other content
types, like Poll, I don't see any indication
of an "Outline" tab.
For me, I have "view", then "edit", then "outline", and it only shows up on non-book content types.
