Thank you vrao. The strange part is that the first time I noticed this happen, it was approximately 3 out of 14 images that were added to my gallery using the image module. The most recent occurrence was with the attachments module, where a document "vanished".
However, your response has been of the greatest assistance.

upload has a few bugs. The 'list' field in table 'files' sometimes get reset to 0.
In addition, when you do a node copy, the files dont get copied. you have to use drupal's built in file_copy.
If you are not very familar with php, I suggest using the 'attachment' module. However, beware that a lot of contributed modules assume you are using upload.

I have found that some, not all, of the files I upload, (eg: photo's, and files attached to blog entries) seem to simply vanish from the system. I can double check after posting that they are indeed present, and a few hours later they are gone.
Watchdog reports no errors, the files simply vanish from the database.
Any ideas?
