You'll probably need to piece some of this together via custom code, but:
Webform captures IP addresses when forms are submitted - webform also can do custom redirects on form submission.
Feedback and Browscap also do some capturing of browser data.

What about detecting their IP address, date, browser, etc. and have that info dumped into a file that could be retrieved at a later date? I know that info is in the server logs, so there must be some way to do it through Drupal? Leslie

Have you seen modules like "Terms of Use" and "Agreement"? They'll do the "I agree" part of your requirement. Not sure if they do the statistical logging you need.
