How do you convert Drupal 6 custom modules and Drupal 6 3rd party custom modules that don't have a Drupal 7 version to a Drupal 7 compatible version?

The Coder module will do quite a bit of it. There is also a long list on DO of the things to check. Otherwise, you have to do it the same way the maintainer would. Please contribute back your code.
