In my case it was not 4.6 and not permissions. So it would be handy to research this a bit further. The moment I go to private files the problem that Sten experienced is also the one I experience.

Sorry I didn't post this to the list (I wasn't sure since I was suggesting 4.6 based fix), but sten's problem was permissions on the files/images folder. (It did not include execute permissions on the directory). It's easy to set up umask to not include read and execute. If that's the case, when the images module creates the images folder it will not be of the correct permissions.

It seems that your images is set to private downloads. I had a similar issue before, and to overcome this I had to switch images back to public downloads. I thought it was a problem with inline.module, but apparently it is not - so perhaps this is a bug in Drupal's code? Can someone else confirm this?
My case was with a custom module I wrote which implemented inline.module, and that caused the same problem as Sten is experiencing when files were marked as private downloads.
